What Is Refractive Lens Exchange?
RLE is a vision correction procedure for patients 45 and older. The eye’s natural lens is removed and replaced with a custom-selected intraocular lens (IOL) to reduce or eliminate the need for glasses, contacts and reading glasses.
The vision changes many people notice after their mid-40s come from the eye’s natural lens, not the cornea. That’s why LASIK, which reshapes the cornea, isn’t always the answer at this stage of life. RLE treats the lens itself.
Depending on your lifestyle and vision goals, lens options include monofocal, multifocal and extended depth of focus IOLs. Dr. Ginsberg helps you choose the lens that fits how you want to see, whether that’s reading a menu, playing golf or driving at night.
Is RLE Right for You?
RLE may be a good fit for Marco Island residents who are:
- 45 or older and want freedom from glasses, contacts or reading glasses
- Living with presbyopia, nearsightedness, farsightedness or astigmatism
- Noticing signs of Dysfunctional Lens Syndrome, such as cloudy or less vibrant vision and trouble seeing at night
- Previously told they are not LASIK candidates
- Patients who had LASIK years ago and feel it has “worn off” (usually a sign the lens is changing, not the cornea)

Why Vision Changes After 42: Dysfunctional Lens Syndrome
Starting around age 42, the eye’s natural lens begins to stiffen and lose focusing power. Ginsberg Eye uses the term Dysfunctional Lens Syndrome (DLS) to explain this gradual change in three stages:
- Stage 1 (ages 42–50): The lens stiffens and loses focusing power, and reading glasses become necessary.
- Stage 2 (50 and older): Light scatter, reduced contrast and weaker night vision.
- Stage 3 (65 and older): A full cataract with poor visual quality.
Patients in Stages 1 and 2 may be candidates for RLE. Understanding where you are on this continuum makes it easier to choose the right option.

What to Expect on the Day of RLE
RLE at Ginsberg Eye takes about 10 minutes per eye:
- You receive a mild oral sedative and numbing eye drops. There is no IV sedation.
- Using an operating microscope, Dr. Ginsberg uses ultrasound and a femtosecond laser to gently remove the natural lens.
- Your new IOL is implanted. The small opening closes without stitches.
Each eye is treated about one week apart. Vision usually improves right away and continues to stabilize within a few days, and everyday activities like driving are usually resumed soon after. Ginsberg Eye will schedule your follow-up visits with you after your procedure.

Frequently Asked Questions About RLE near Marco Island
Where is RLE performed for Marco Island patients?
RLE is performed in our in-office surgery suite at the Ginsberg Eye Naples location, about 18 miles from Marco Island.
Can I have my consultation in Estero instead?
Yes. Consultations are available at both our Naples and Estero offices. The procedure itself is performed in Naples.
Is RLE the same as LASIK?
No. LASIK reshapes the cornea and is typically for patients 18 to 45. RLE replaces the eye’s natural lens with an IOL and is typically for patients 45 and older.
Does RLE hurt?
RLE is painless. You’ll receive a mild oral sedative and numbing eye drops, with no IV sedation.
How long does RLE take?
About 10 minutes per eye. Each eye is treated about one week apart.
Can I finance RLE?
Yes. CareCredit financing is available for refractive lens exchange, subject to credit approval. Patients may also use FSA or HSA funds. Learn more on our financing page.
My LASIK seems to have “worn off.” Could RLE help?
Often, that change is the natural lens aging, not the cornea. RLE treats the lens itself, so it may be an option. A consultation at Ginsberg Eye will tell you for sure.
